Cardiac troponin I (cTnI) is a protein with a molecular weight of 22.5 kDa, which is found in the heart muscle. Troponin I is a component of a structure, which also consists of troponin T and troponin C. Together with tropomyosin, this structure forms the main component that regulates the calcium-sensitive ATPase actomyosin activity in transversely striated skeletal and cardiac muscles.
Following cardiac injury, 4 – 6 hours after the onset of pain, troponin I released in the blood. The release of cardiac TnI resembles that of the CK-MB, while CK-MB returns to the normal level after 72 hours troponin I remains at increased levels for more than 6 – 10 days and thus offers a longer time frame for detection of cardiac injury. The high specificity of cTnI measurements for detection of myocardial damage was shown in surgical interventions, after marathon runs and blunt chest trauma. Distributions of cTnI could also be documented in pains other than acute myocardial infarction (AMI), such as unstable angina, congestive heart failure and ischaemic damage due to bypass operations of the coronary arteries. Because of its high specificity and sensitivity in myocardial tissue, troponin I has lately become the preferred biomarker for myocardial infarction. The MediNOW troponin I test is a simple quick test, with which a combination of particle-conjugated cTnI-specific antibodies and receptor reagents can be selectively detected in cTnI in whole blood, serum or plasma. The detection limit is 1 ng / ml.

The MediNOW troponin I test is an immunoassay, which qualtitatively detects cTnI in whole blood, serum or plasma. The membrane is coated in area of the test line with receptor reagent. During testing, the cTnl contained in the whole blood, serum or plasma sample reacts with the two specific anti-cTnI antibodies of the assay, one of which transfers the binding to the receptor reagent and the other is colour-coded. The mixture traverses the membrane due to the capillary force. In the test linear region, the cTnl antibody complex of the immobilised receptor reagent is intercepted so that a red line appears.
This indicates a positive result. If the sample does not contain cTnI, no line appears in the test region and thus, indicates a negative result. Moreover, in the control line region (C) of the test, a red line, which is formed independently of the cTnI concentration in the sample material, should appear. It serves as a function check and indicates that a sufficient amount of sample was used and that the liquid has penetrated the membrane completely.
